POSITION SUMMARY:
Under the direction of the San Diego PACE Medical Director, the PACE Physician provides comprehensive medical care to the PACE participant. The PACE Physician attends to the acute needs of the PACE participant, while managing the chronic medical problems, and paying attention to the preventive and rehabilitative care required by the PACE participant. The PACE Physician incorporates the entire interdisciplinary team as appropriate to provide the medical care in a holistic manner, as is the hallmark of the delivery of care to the PACE participant.
