We're looking for a driven and detail-oriented professional to join our team as a CHW School Based Navigator. In this role, you'll play a vital part working within the Mansfield City School System to identify students that can benefit from social determinant interventions that may be affecting student health and academic performance, helping us move forward with purpose and precision. Essential Job Duties: Communicates to clients the purpose of program services and the potential impact of those services on their health and wellbeing. Networks with city officials, housing providers, and social service agencies to place unhoused individuals in a stable housing environment. Coaches clients in the effective management of chronic health conditions and self-care, while motivating them to be active, engaged participants in their overall health. Conducts patient outreach activities, including cold calling, to encourage patients to schedule appointments for care gap closure. Works effectively with individuals from diverse backgrounds in reducing cultural and socio-economic barriers between clients and available institutional resources. Build community capacity by addressing the social determinants of health and identify individual and community needs. Successful completion of a Community Health Worker formal training program such as from a college or other education institution within one year of date of hire. Maintain the certification once earned. Ability to be insured on the commercial auto insurance policy based on driving record including active driver’s license and personal automobile insurance and reliable transportation. Employment offers conditional upon completion of a background screen including fingerprinting and a drug screen Within the first 90 days of employment: BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation.
