About the Role As a Client Care Coordinator, you’ll serve as the key connection between clients, families, and our clinical and case management teams. You’ll guide clients through the intake, treatment, and discharge process, ensuring their needs are met with empathy, professionalism, and care. This position is ideal for someone with experience in healthcare administration, behavioral health, or substance abuse treatment coordination, who enjoys creating structure, building relationships, and supporting people during challenging times. Key Responsibilities Act as the primary point of contact for clients and families throughout their care journey. Coordinate client intake, discharge planning, and follow-up care. Communicate with clinical and case management teams to ensure individualized client support. Maintain accurate, organized documentation in accordance with facility policies.
