We are seeking a Client Care Manager to join our care team. The Client Care Manager receives and manages potential client inquiries and designs customized care plans to support and meet the needs of the client, client family, client representatives and significant others. Reviews the services provided to the client, makes home visits and conducts ongoing audits of client care plans. Supports, guides and provides direct supervision to in-home caregivers to assure they understand and meet the unique needs of each client. The Client Care Manager, along with the Client Care Coordinator, provides specific training to caregivers as necessary. The Client Care Manager will, at times, be called upon to provide hands-on care as needed or deemed necessary by the Agency. The Client Care Manager will also participate in the after-hour phone on-call rotation.
