CANDIDATE REQUIREMENTS

  • At least 2 years of experience in a Care Management role.
  • Strong knowledge of CQC regulations and KLOEs.
  • Proven experience writing care plans and risk assessments.
  • Good IT skills, including Outlook, Excel, and experience with care management systems.
  • Minimum of 5 GCSEs (A-C), including Maths and English.
  • Must live locally to New Malden, hold a driving licence, and have access to a vehicle.
  • Excellent communication, organisational, and time management skills.
  • Professional, caring, and positive approach to work.
Responsibilities

THE ROLE

As Client Care Manager, you will be responsible for managing your own portfolio of clients and ensuring they receive safe, person-centred care of the highest standard.

Key duties include:

  • Managing client portfolios, ensuring compliance with company policies, CQC regulations, and best practice standards.
  • Conducting risk assessments, care planning, service reviews, quality assurance visits, and MAR chart updates.
  • Overseeing hospital discharges and liaising with multi-disciplinary teams to ensure smooth and safe transitions home.
  • Building strong working relationships with healthcare professionals, such as GPs, district nurses, pharmacies, and social workers.
  • Supporting families with professional guidance and reassurance.
  • Working closely with the internal team to match Care Professionals appropriately to client needs.
  • Promoting the highest standards of service delivery with a focus on compassion and person-centred care.
