At Senior Home Companions (SHC), we’re on a mission to create active and engaged lives through exceptional care — and it all begins with our Care Managers. We are filling this role in our Punta Gorda Office and will be servicing the SW Florida market. What you’ll do: As a Care Manager, you’ll be the vital link between clients, families, and caregivers. You’ll build lasting relationships by: · Understanding and tailoring home care solutions to each client’s unique needs. · Coordinating caregivers with thoughtfulness and attention to detail. · Communicating and documenting important caregiver and client feedback. · Supporting our team to deliver care that not only meets but exceeds expectations. Requirements What we’re looking for: We want someone who is: · Compassionate, personable, and relationship-driven. · An excellent communicator, both written and verbal. · Highly organized, flexible, and skilled at prioritizing. · A natural problem-solver who thrives in a dynamic role. · A background in customer service, preferably in a health care setting. · Professional demeanor and polished appearance.
