Isaiah House is looking for a dedicated and driven person to take on the role of Client Care Navigator in the Lexington, KY. A Client Care Navigator (CCN) is a very important role within the Isaiah House organization. Navigators represent the treatment programs offered by Isaiah House to front-line referral agencies throughout the state of Kentucky. Requirements JOB SPECIFIC FUNCTIONS: Build strong relationships with Centers of Influences (COI) in the CCN assigned Region Educate COIs on the history, programs and outcomes provided by Isaiah House Field Calls from potential clients who are exploring treatment options Manage travel schedule to counties specifically assigned Maintain database on all COI activity, Client referrals, Clients admitted in ASANA Communicate all new client referrals through CCN supervisor via MS Teams QUALIFICATIONS: High school diploma or GED preferred Knowledge of addiction and substance abuse treatment Connections with community services, organizations, and other public groups is essential If in recovery, must be able to demonstrate one year of continuous sobriety
Responsibilities
The Client Care Navigator represents Isaiah House treatment programs to front-line referral agencies across Kentucky and builds strong relationships with Centers of Influence within their assigned region. This role involves educating these contacts on the organization's history, programs, and outcomes, while also managing field calls from potential clients.
