Description The Client Coordinator works closely with clinicians, other MHTs, and administration staff to aide getting the clients checked in and checked out of the detox facility. This position focuses on the admissions process and completing the discharge process. The Client Coordinator will also provide case management services through the development of discharge planning and all the components that allow for smooth coordination of care. Duties/Responsibilities · Actively participate as a positive member of an interdisciplinary treatment team. · Complete accurate documentation same day as date of service. · Maintain good standing with outside (referral) agencies to be able to provide placement options to our clients · Stay current with updated research and best practices. · Be aware of community referral sources and refer when necessary. · Secure client information per HIPAA standards. · Provides and attends in-service training as needed. · Complete case management functions focused on a housing first and high needs case management model. · Complete AHCCCS application and employment specialty services paperwork. · Develop client-driven discharge plans and continually monitor and record discharge plan progress. · Collaborate with peer employees, other housing staff and management to ensure fidelity to program structure and guidelines. · Assist clinical, housing, and support staff in coordinating care with external sources. · Works as part of highly communicative treatment team and supports staff in promoting treatment goals and values of the Axiom Care program. · Complete documentation in the electronic medical record of activities, interventions, client contact, client staffing and case management treatment planning. · Attend frequent site meetings with peers, clients (house meetings), and ongoing staff trainings. · Transport clients to appointments as needed. · Obtain Vital signs, UAs, and BAL. · Create ROIs for clients’ HIPPA protection. · Communicate with the RNs and assist with any patient needs. · As part of Axiom Care’s commitment to Culturally and Linguistically Appropriate Services (CLAS), this position supports efforts to provide inclusive and accessible translation services for clients.
