Catholic Charities of Los Angeles, Inc. is one of the largest human services providers in California that operates a variety of programs to serve the poor and strives to find permanent solutions to crisis situations by offering clients the tools and resources needed to achieve greater self-reliance and stability in their lives.
St. Margaret’s Center is a program of Catholic Charities of Los Angeles that serves as an access center for the L.A. County’s Coordinated Entry System (CES) in the northern part of the South Bay. St. Margaret’s Center is seeking a Client Resources Coordinator (CRC) III that will work as part of the homeless services team to implement a Housing Focused Case Management Program. Under the direction of the Homeless Services Manager, the CRC III is responsible for providing individualized case management and street outreach to households experiencing homelessness in L.A. County’s Second District (SD2) utilizing a theoretical framework incorporating Trauma Informed Care, Motivational Interviewing, Critical Time Intervention and Stages of Change. The program service area focuses on the northern area of Service Planning Area (SPA) 8 including Lennox, Del Aire, Westmont/West Athens, Inglewood, and Lawndale.
The CRC III will perform specialized tasks including comprehensive case management services such as intake, assessment, goal setting, monitoring and reassessment, life skills, referrals and linkages to other providers. The CRC III will also perform street outreach functions, such as responding to LA-HOP requests, and serve as a housing navigator for unhoused households to ultimately secure bridge and permanent housing. The CRC III will document all participant interactions and input that data into the Homeless Management Information System (HMIS/Clarity).
This position requires the skills and willingness to be successful at every step of the homeless rehousing process—from street outreach to housing navigation to permanent housing and retention.

QUALIFICATIONS:

  • Bachelor’s degree required.
  • Three to five years of related experience working with people experiencing homelessness, especially within the Los Angeles County CES system.
  • Valid California driver’s license and valid auto insurance.
  • Equivalent combination of education, training, and experience acceptable.
  • Bilingual (English/Spanish) preferred.
  • Can demonstrate leadership abilities.
  • Thorough knowledge of the CES system and general knowledge of homeless services in Los Angeles County.
  • Knowledge of Harm Reduction, Housing First and Trauma-Informed Care models of service delivery.
  • Basic understanding of severe mental health, physical health, and substance use issues and symptoms.
  • Administrative principles and their application.
  • Social services resources and governmental agencies and programs.
  • Applicable state and federal laws and regulations.
  • Outcome frameworks and metrics.
  • Catholic Charities’ policies, procedures, and organization.
  • Database software; Internet software; Project Management software; Spreadsheet and Word Processing software; HMIS/Clarity.
  • Passion for ending homelessness.
  • Ability to perform street outreach and crisis case management services in challenging environment.
  • Maintaining a non-judgmental attitude and a display of unconditional positive regard when working with highly vulnerable/challenging individuals.
  • Strong people skills and ability to work effectively with people of varying racial, ethnic, cultural, educational and socioeconomic backgrounds.
  • Ability to listen with sensitivity to other people’s feelings, needs, and points of view; demonstrate tact and courtesy in expressing opinions or ideas, projecting a positive image of St. Margaret’s Center/Catholic Charities of Los Angeles.

Responsibilities:

  • Assesses participants’ needs and helps them address and achieve their identified goals.
  • Conducts a CES Assessment for each participant.
  • Develops problem-solving relationship with participants. Assesses and identifies their needs and abilities.
  • Participates in street outreach programs with SPA-wide team members.
  • Develops a Housing Services Plan to fulfill assessment goals.
  • Coordinates with agencies/programs to ensure participants receive all needed services.
  • Maintains proper documentation and current case notes in HMIS/Clarity computerized database system and in participant files.
  • Monitors cases and plans for re-assessment.
  • Maintains confidentiality of participant information and records.
  • Serves as client advocate with government agencies to obtain public assistance.
  • Presents participant cases at case conferencing meetings.
  • Displays sensitivity to the participant population’s cultural and socioeconomic characteristics
  • Develops relationships with housing providers, property owners and property managers in order to navigate participants into permanent housing.
  • Promotes continuous quality improvement and service delivery that is aligned with the Agency’s mission.
  • Handles public contact and relations effectively.
  • Plans short- and long- range activities.
  • Performs related duties as required.
