With experienced colleagues, approachable leaders, and a supportive environment, you’ll have everything you need to thrive and develop your skills.   Take your next step with HenderCare  About the Role As the Client Support Partner, you will be responsible for working with new and existing clients who are living with a disability to support them by the development and implementation of care plans, participant goal setting, and reablement strategies to ensure that our clients receive quality, person-centered care that enhances their independence and wellbeing and promotes their positive outcomes.    You're responsibilities will also include:   Developing, reviewing, and maintaining individualised care plans that reflect participant goals, preferences, and funding eligibility. Working closely with clients, families, and support networks to ensure services align with reablement and wellness principles. Conducting quarterly budget reviews and ensuring services remain within allocated budgets. Undertake regular reviews of service agreements. Performing risk assessments relates to client’s health, home environment, and personal safety. Assisting participants in understanding their approved services, funding classifications, and rights within the relevant framework. Promote HenderCare services to potential clients and referral sources to increase service capacity.  Our Ideal Candidate We are looking for engaging, energetic, and passionate people who show a strong commitment to the overall growth of the organisation. Attention to detail, and having the capacity to absorb and apply new information and processes will allow you to succeed in this role. You will need to be able to show initiative and have confidence in your own skills and abilities, including being able to prioritise your workload and work with tight deadlines with relative autonomy in what can be a challenging environment.   You will also bring:   At least 2 years of relevant experience in NDIS Case Management. Demonstrated experience in disability case management, or social work. Knowledge of relevant funding models  Strong understanding of the wellness and reablement approach and its application. Experience working with vulnerable and diverse client groups, including those with dementia, disability, or mental health challenges. Certificate IV in Aged Care, Disability or Community Services (Desireable)     Only applicants with the right to work in Australia will be considered.
