Responsibilities Manage patients in the ambulatory care setting through a collaborative, team-based model involving providers, clinic staff, resource specialists, and clinical resource coordinators. Provide disease state management services to support chronic condition care and therapeutic outcomes. Deliver patient education to improve understanding, adherence, and self-management of prescribed therapies. Optimize drug therapy regimens in partnership with providers, ensuring safe, effective, and cost-conscious medication use. Contribute to the design and implementation of clinical initiatives and care models in collaboration with clinic teams. Qualifications Education: Graduate of School of Pharmacy. Advanced degree (Doctor of Pharmacy preferred) with residency training or equivalent work experience required. Experience: Experience in ambulatory setting with disease state management preferred. Licensure Current applicable state Registered Pharmacist License (RPh) required. Pay Range USD $80.78 - USD $93.71 /Hr.
