Responsibilities

Join a high performing multidisciplinary team, where you will provide high level clinical assessments. The role of the Clinical Aged Care Needs Assessor is to utilise the Integrated Assessment Tool to determine the care needs of frail older people in any setting and facilitate access to available care services appropriate to their care needs.

As the Clinical Aged Care Needs Assessor, you will be responsible for:

  • Undertaking comprehensive Aged Care clinical assessments.

  • Working collaboratively within a multidisciplinary team to optimise the health care and wellbeing of each client and their loved ones.


  • Coordinate and facilitate access to the most appropriate type and classification of aged care and other support services that best meets the Client’s current, assessed care needs.

  • Supporting complex cases with a variety of challenges and considerations to navigate
