Smarter Connections is seeking a Clinical Care Coordinator to support the delivery of high-quality, in-home aged care services across Melbourne’s northern suburbs.

ABOUT THE ROLE

As a Clinical Care Coordinator, you will play a key role in supporting older clients to live safely and independently at home. You’ll be responsible for conducting assessments, developing and monitoring care plans, providing clinical oversight, and guiding a team of support staff to ensure excellent client outcomes.
This is a casual role (with potential to transition to permanent) ideal for an experienced Registered Nurse (minimum 2 years in aged care setting) who is flexible, compassionate, and enjoys working both independently and as part of a dedicated care team.

KEY RESPONSIBILITIES

• Conduct clinical assessments and reviews for aged care clients
• Develop, implement, and update individual care plans
• Provide clinical advice and mentoring to support staff
• Monitor client outcomes and escalate changes in condition
• Liaise with families, GPs, allied health, and aged care brokers
• Ensure care is delivered in line with the Aged Care Quality Standards and organisational policies
