THE OPPORTUNITY:

Join the innovative, dynamic and supportive management team at McIntosh Court – Booragul as the Clinical Care Coordinator. This is a great next step for a passionate Registered Nurse with experience in an Aged Care environment.
Work closely with the Residential Care Manager to ensure the overall clinical management of consumers are in line with the Aged Care Quality Standards, and other relevant legislation, to ensure we continue to meet the quality needs of our consumers.
This is a Full Time position – Monday – Friday
To be considered, you’ll need current AHPRA registration and proven clinical experience within the aged care sector.

Responsibilities
  • Organising and facilitating case conferencing to partner with consumers and their representatives in the development of care provisions and plans;
  • Be a clinical care leader and go-to contact within the home
  • Provide expert case management alongside the Residential Care Manager
  • Monitor and improve clinical outcomes through audits and quality initiatives
  • Support policy development and implementation for resident care
  • Lead, coach and inspire staff to deliver outstanding car
