As a Clinical Coordinator, you’ll play a vital role in supporting the Care Manager to ensure residents receive the highest quality of care. You’ll help lead a dedicated team of nurses and care staff, coordinate clinical practices, and foster a culture of respect, empathy, and excellence — making a meaningful difference every day. This is a permanent full-time role based in Anglicare Porter Lodge, Minto Your opportunity Empathy & Respect: Treat all residents with empathy and dignity, providing them with choices to live their daily lives in a way that is special and unique to them. Effective Coordination: Monitor and correct staff practices, ensuring the effective coordination of residents’ daily clinical and personal care needs. Care Planning: Establish, monitor, and update care plans by reviewing, discussing, developing, and writing them for residents. Leadership: Provide professional, aligned, and capable leadership to the team, embodying the organisation’s Christian vision, mission, values, and strategy. Clinical Outcomes: Oversee the activities of RNs, allied health professionals, and doctors to ensure clinical benchmarks are achieved and Anglicare policies are followed. Multidisciplinary Team: Provide accurate and complete information to the multidisciplinary team to ensure they can perform their duties effectively. Funding: Monitor documentation to optimise funding, ensuring all required information is complete, accurate, and timely. Team Functioning: Encourage teamwork, provide advice and guidance, assist in performance reviews, and participate in the selection of RNs. Family Communication: Ensure families are fully informed about clinical issues, provide emotional support, coordinate counselling, and escalate issues as necessary. Resident Admission: Oversee the initial assessment process to ensure suitable residents are admitted in the right category. What you’ll bring Experience as a Registered Nurse, AHPRA registered- mandatory Previous experience in Residential Care Services Demonstrated clinical expertise, with contemporary clinical practices, thorough understanding of aged care standards, and evidence-based procedures to support safe and effective healthcare delivery Strong communication skills (written and verbal) and ability to build rapport with residents, their families and staff Ability to communicate effectively with all levels of staff and management with demonstrated experience in being respectful and collaborative to foster positive relationships Proven ability to coach and guide team members while effectively managing day-to-day responsibilities and operational demands Sound judgment in knowing when to escalate issues, after applying experience and problem-solving skills to resolve them independently Intermediate computer skills and experience working in an electronic environment.
