Clinical Care Coordinator  * Progress with purpose at one of Australia’s largest health care providers * Get ahead with salary packaging, benefits and professional development in a supportive team environment * At least 3 years’ experience in aged care.  About the role As a Clinical Care Coordinator, you'll lead our clinical and care teams to ensure residents receive exceptional care while nurturing an understanding of their unique identities and experiences. You'll also play a pivotal role in our home's clinical governance and may manage specialized clinical portfolios, such as education, palliative care, and infection control. Your main duties will include: * Ensure comprehensive clinical and specialized nursing care is provided, documented, and tailored to each resident's needs by our clinical staff. * Prioritize residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks. * Oversee and conduct all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required. * Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly. * Conduct routine clinical assessments to promptly address identified gaps in care. About you: * Currently registered as a Nurse with AHPRA, with no restrictions. * Demonstrated management experience in aged care sector  * In-depth knowledge of the aged care industry, with a keen eye for emerging trends and best practices. * Proficiency in managing human resource matters, complemented by strong negotiation skills. * Skilled in utilizing various information technology tools and software, including Word and Excel, with a quick ability to adapt to new skills as required.
