Description Clinical Care Coordinator General Summary: Responsible for developing and implementing referral Care coordination, sales, and marketing strategies that achieve territory market growth objectives and strategies of Home Health. Works in partnership with the Director of Business Development to develop effective area sales support with a representative of Lifecare Home Health Family/Complete Home Care business interests in the market. Responsible for establishing referral relationships, support, and patient care coordination of referrals and partnership opportunities with major area referral sources, Bundle ACO Partnerships, and contracts. General Responsibilities: • Ensures comprehensive information is gathered continually regarding assigned area market, customers, and competition to determine the current situation and identify trends and opportunities to propel the goals of the territory, and patient demographics & orders, respectively. • Executes collection of referral/patient details, face-two-face, demographics, orders, H&P, and progress notes with all referrals for assigned territory in concert with supervisor, DBD • Develops and implements sales and marketing strategies that exploit area market opportunities in coordination with the DBD/supervisor. • Cultivate key partnership/contract opportunities with metro area Hospitals, Physician groups, local Accountable Care Organizations, Bundle contract opportunities, and strategic alliances to drive organic growth in episodic admissions in support to their direct supervisor. • Develops referral relationships with major accounts assigned to, including large hospitals and health systems, major physician practices, and other potential large referral sources. • Identifies, develops, and implements new products, services and referral relationships that take advantage of market opportunities consistent with metro marketing strategies. • Analyzes referral patterns and trends and communicates findings to leadership to assist in revising the territory marketing strategies. • Prepare special market studies and research as needed in support to DBD. • Meets or exceeds established productivity goals and goals for episodic census growth. • Represents the company appropriately to groups, community organizations, and the public. Requirements Education: Bachelors Degree Preferred Required: LPN Qualifications: Strong oral and written communication skills, Excel, Word, and CRM management. Excellent presentation skills. Experience: 3-5 years in patient care, sales, or related field; demonstrated proficiency in-home care marketing/sales preferred. Physical Demands: Regular local travel (weekly) in all reasonable weather conditions. Primarily medium in nature with exerting up to 35 pounds of force to lift, carry, pull, or otherwise move objects; involves sitting, walking, standing, driving, talking, hearing, and reaching. Working Conditions: In and out of offices, referral sources, and care throughout the workday. Exposure to disagreeable conditions, including odors, infection, illness, and physical contact with disruptive patients. Work Schedule: Must be flexible to accommodate the completion of tasks, operational needs, and referrals.
