Right at Home – Northern Rivers is seeking a Full-Time Registered Nurse – Clinical Care Coordinator to join our dedicated and growing team in Ballina. This pivotal leadership role combines clinical nursing, care coordination, and relationship management to support elderly, frail, and disabled clients with person-centred, high-quality care.

MINIMUM REQUIREMENTS

  • Current AHPRA Registered Nurse certification
  • National Police Clearance
  • Australian Driver’s Licence and reliable, insured vehicle
  • Strong verbal and written communication skills
  • Knowledge of aged care frameworks, including the Aged Care Quality Standards

DESIRABLE QUALIFICATIONS

  • Postgraduate certification in geriatric nursing, dementia care, or palliative care
  • Tertiary qualifications in health, aged care, or management
Responsibilities

ABOUT THE ROLE

As a Clinical Care Coordinator, you’ll be instrumental in delivering consumer-directed services that promote independence, dignity, and wellbeing. Your day-to-day will include:

  • Conducting clinical assessments and developing tailored care plans
  • Overseeing the coordination of an allocated case load of client services and supporting care staff
  • Providing direct nursing care when required
  • Identifying and managing clinical risks with a proactive approach
