The Clinical Care Coordinator also functions as a multi-skilled health professional who works in cooperation with other health care professionals to provide quality health care to the patient. Description of Primary Responsibilities Responsible for patient care Provide necessary medical services to patients of Palms Medical Group (PMG) within his/her scope of practice in accordance with stated policies, procedures and protocols of PMG. Maintain the Electronic Health Record (EHR) with up to date documentation of services provided. Provide counseling within scope of practice, reinforce education provided to patients by the provider or Clinical Nurse Educator and administer patient educational materials as directed by PMG providers. Triage nurse visits and return phone calls to patients and vendors while maintaining communication within scope of practice. Responsible for inventory management Maintain knowledge of inventory procedures and submit any order requests to the VP of Business Continuity. Restock examination rooms, lab and clinical supplies as needed. Maintain orderly and professional work areas. Responsible for after hour nurse duties Communicate with patients in a professional and courteous manner. Document and update the patients EHR as soon as possible (no later than the next business day) following the telephone communication. Task and inform the appropriate provider of the communication. Will be expected to provide and use personal cell phone during on-call duties. Inform nursing supervisor when starting and completing scheduled on-call duties. Responsible for administrative duties Direct, supervise, train and evaluate employees. Utilize the Leadership evaluation manager (L.E.M.) to promote goals for each quarter. This includes creating and following up on action plans each quarter and monthly rounding’s with each employee. Attend LEM leadership meetings as required Oversee daily clinical activities. Review, edit and submit employee time cards to Human Resources. Grant employee leave in accordance with PMG policy and staffing considerations. Participate in QA/QI and other meetings and/or activities as directed by the Regional Manager or VP of Practice Management. Assist the VP of Practice Management and/or Regional Manager with data collection as necessary. Complete nurse audits through I2I and document via one drive monthly; administer and collect surveys and follow up with red charts as directed by the VP of Clinical Care Coordinators. Establishes staff schedules, allocation of staff, and assures effective patient care. Works with Administration to ensure coverage for Trenton Saturday Clinic and after hours on call. Sets departmental goals and monitors staff progress toward goals. Conduct monthly nursing meetings. Understand and administer HR policies and procedures to ensure staff compliance. Assure staff meets deadlines on projects assigned i.e. annual training, licensure, Relias, and health requirements. Oversee completion of monthly safety inspection, drills, and all logbooks. Complete a bi-weekly report every 2 weeks and send to Region Manager and VP of Practice Management Perform any other duty assigned by the Regional Manager or VP of Practice Management or CEO to improve the efficiency of PMG. Requirements Description of Primary Attributes General Development: Must be organized, self-motivated and detailed oriented. Exhibits strong leadership skills. Demonstrate balance between Clinical Care Coordinator duties and patient care responsibilities. Job duties require the ability to work independently and as part of a team. May be expected to travel to and provide coverage for other PMG clinics during nursing staff absences. Serves as a mentor for nursing staff. Professional & Technical Knowledge: 1. Employee will meet Continuing Education Requirement for his/her certifications and licensures as per Florida State Statutes and/or board of authority. 2. Employee will exhibit competency when utilizing the following equipment: ECG PFT In-house hematocrit and centrifuge In-house hemoglobin Lab centrifuge AED Autoclave Vital Sign Machine Nebulizer and accessories Blood Glucometer INR In-house hemoglobin A1c Other equipment as listed on job competencies. 3. Employee will comply with job competencies and skill check-offs as part of annual evaluation. 4. Employee will be expected to navigate and operate Microsoft Office suite products, including Word, Excel and PowerPoint. Licenses & Certifications: High School Diploma or Equivalent Graduate of an accredited LPN or RN program Current LPN or RN or license in the State of Florida Minimum of one (1) year LPN or RN experience in Primary Care setting preferred.
