The Clinical Care Coordinator is responsible for conducting assessments for all per MDS schedule, support documentation and social care plans geared to restoring residents/patients to their optimum level. Supports residents and their families to utilize the resources internally and in the community, particularly concerning Medicare and Medicaid. This position promotes a positive, calm and effective work environment that contributes to teamwork.  The Clinical Care Coordinator position requires excellent communication skills with team members, residents and family members as well as a high level of patience and compassion for others.     Responsibilities  * Complete resident interviews and assessments. * Provide consultation to members of staff and coordination with community agencies etc. in an effort to solve issues and meet the needs of residents and ensure appropriate care. * Discharge planning for short term residents. * Discharging planning for short term residents.  * Assist with insurance appeals. * Assist residents and their families with personal and environmental difficulties.  * Consults with physician and other members of the care team for the purpose of assisting them to understand significant social, emotional and environmental factors related to the patient’s/resident's health issues.  * Organizes and plans discharge process for long term care residents and for short term/skilled residents moving to different levels of care. * Develop Social Services care plan.  * Assist with clerical tasks. * Participate and schedule care conferences, as part of interdisciplinary team. * Oversee and complete documentation required for Part A and Part B billing; i.e., Notice of Medicare Non-Coverage and Advance Beneficiary Notice. * On-call duties assigned    Requirements * Must possess current LSW or Nurse state license. * Experience in a medical facility, senior living facility preferred, where the team approach to treatment is utilized.  * Microsoft Office proficiency.
