Responsibilities

ABOUT THE ROLE

As a Clinical Care Coordinator you will play a vital role in delivering high-quality clinical care and fostering a culture of excellence across the home. Working closely with the Home Manager, you will lead and support the care and clinical teams to ensure each resident receives personalised, dignified, and safe care that respects their individual identity, culture, beliefs, and life experiences.
This role is ideal for a Registered Nurse with strong clinical experience and with a commitment to continuous improvement, with the ability to coach and mentor staff while contributing to quality and governance activities.

KEY RESPONSIBILITIES

  • Lead clinical care planning, documentation, and delivery to meet the holistic needs of each resident
  • Champion person-centred care and involve residents and families in meaningful care decisions
  • Guide and support clinical staff in AN-ACC assessments and accurate, timely documentation
  • Support and educate Registered and Enrolled Nurses accountable for daily clinical responsibilities, including case reviews and care outcomes
  • Conduct regular clinical audits and ensure prompt follow-up on care gaps or concerns
  • Support a culture of safety, compliance, and continuous improvement across the home
