We are seeking a Registered Nurse (RN) with Clinical Care Coordinator experience to join our team!

REGISTERED NURSE (RN)/ CLINICAL CARE COORDINATOR REQUIREMENTS:

  • Current, unrestricted California RN license
  • Min. 1 year experience as RN in Sub-Acute unit.
  • Min. of one year experience within the past two years working in a general acute care facility or an acquired equivalent competency appropriate to the type of sub-acute patient for which the facility provides care.
  • Preferred but not required to have knowledge of OBRA and Title XXII guidelines for LTC
  • Current BLS (CPR) Training.

SUMMARY:

The Clinical Care Coordinator (RN) is responsible for the day-to- day nursing activities on the Sub-Acute unit and for the quality of resident care delivered in that unit. As the CCC of the Sub-Acute unit, the CCC is responsible for the overall management of the sub-acute unit. The CCC reports to the Director of Nurses and/or the Facility Administrator. The position involves 20% direct patient care and 80% administrative, non-direct patient care. The CCC assists in providing a clean, safe, dignified, happy environment for residents by performing the following duties:

Responsibilities

PRIMARY RESPONSIBILITIES:

  • As a member of the QA Team with specific responsibilities related to evaluating compliance in the Medi-Cal Certified Sub-Acute Units.
  • Under the direction of the VP of Clinical Services, performs tasks that include, but are not limited to, evaluating and assessing the performance of the CCC’s in the sub-acute units.
  • The Sub-Acute CCC Nurse Consultant assists in providing a clean, safe, dignified, happy and healthy environment for residents.

SUPERVISORY RESPONSIBILITIES:

  • This position participates in interviewing and hiring for the Subacute Unit; planning, assigning, and directing staff; evaluating performance for initial and annual evaluations; counseling and disciplining employees; addressing complaints and resolving problems; and recommending employment decisions including terminations.

OTHER ESSENTIAL RESPONSIBILITIES:

  • Complies with the facility’s attendance policy requiring regular, reliable, and punctual attendance.
  • Orientation of all new CCCs.
  • Participates in training of sub-acute staff, including ventilator training and seminars.
  • Participates as member of Quality Assurance Team, attending monthly QA meetings.
  • Participates in annual QA visits to facilities as member of QA Team with assessment of CQI Standards specific to Sub-Acute Units.
  • Visits all sub-acute units on a regular basis with quality-of-care rounds to assess status of overall subacute care provided, including ensuring that appropriate equipment is available/in use, and exit with Administrator, DON and CCC to review findings and recommendations.
  • Reviews clinical records for high risk residents to ensure documentation is reflective of appropriate interventions.
  • Ensures that overall quality of care, residents’ rights, State Sub-Acute policies/procedures, infection control, condition change, treatments and NHPPD (3.8 licensed/2.0 C.N.A.) are met.
  • Assists with development and implementation of policies and procedures.
  • Ensures that facility policies and procedures, including documentation are followed. Provides in-service education to sub-acute staff as needed, including training based on findings of visit, including, but not limited to:o Infection Control.
  • Documentation of Condition Changes and Physician Visits in a timely manner
  • Ensures that mandated logs are in place.
  • Reviews and ensure that State (Title XXII, Division V, Chapter III), Medi-Cal Sub-Acute Guidelines and Federal guidelines are followed.
  • Assists in recruiting new CCC’s and other sub-acute staff as needed.
  • Assists with marketing to improve census that may include meeting with physicians as needed.
  • Works closely with Director of Respiratory Services to ensure continuity of sub-acute care.
  • Other duties as assigned by the Administrator and Director of Clinical Services.
