Discounts on wide array of services/entertainment nationwide The Clinical Care Manager is responsible for: Collaborating with team members to implement measures that decrease episodic care and meet quality outcomes. Supervising team of care coordinators to help MassHealth members connect to the appropriate care. Reviewing client’s care plan with team members and other stakeholders regularly. Working as part of a multidisciplinary team consisting of Behavioral health, Medical, Recovery support staff, to help navigate and coordinate care for clients exhibiting complex health issues. Participating in meetings with other Community partners, and stakeholders. Supporting care coordinator in advocating for client treatment. Developing a person-centered treatment plan with each Engaged Enrollee, with input from Care Team members and other stakeholders as well as updating the treatment plan according to required timeframes. Supervising Care Coordinators in supporting Engaged Enrollees during care transitions including attendance at discharge planning meetings, face to face meetings post discharge, and facilitating enrollee participation in those services. Meeting expectations related to supervising the programmatically required number of Care Coordinators and supporting the programmatically required number of Enrollees which may vary over time. Conducting weekly supervision with assigned care coordinators. Learning all BHCP policies, procedures, protocols and plans and deliver CP supports and activities in compliance with them. Ensure all CP Team members are trained and comply with these protocols. Developing knowledge about all focus populations and assist CP Team members in doing so. Learning evidence-based practices identified by the BHCP, and deliver CP supports and activities in compliance with them and ensure all CP Team members are trained and supported in implementing them. Qualifications Qualifications: RN or Masters degree with first level license (RN, LICSW, LMFT, LMHC, licensed psychologies, or LADAC I) with minimum of 2 years of experience working in a community-based mental health, substance use and/or physical health setting. Minimum of 2 years’ supervisory experience, preferred. Demonstrated knowledge base and judgment necessary for independent clinical decision making. Minimum of 3 years professional and clinical experience in the areas of mental health, counseling, and substance use. Strong skills in the areas of communication, follow through, collaboration, and customer service, time management and organizational skills. Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations and understand the social drivers of health (SDOH) needs associated with these populations. Excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and patients.
