We are seeking a Clinical Care Partner to support safe, coordinated and proactive care for customers with complex or changing needs.

This role is a key clinical position within the Neighbourhood Care Partnering model, providing clinical leadership, escalation management and decision support to Care Partners and the broader multidisciplinary team. The Clinical Care Partner plays a critical role in identifying and managing clinical risk, supporting customer outcomes and ensuring customers receive the right care at the right time.

This is a Permanent Full-Time opportunity based in Grafton or Maclean offering flexibility and a great work-life balance.

• Flexible work hours: 8:30am – 4:30pm or 9:00am – 5:00pm, Monday to Friday
• Remuneration ranges from $105,000 to $120,000 TFEC
• Hybrid working model combining client visits, office days & work from home opportunities.
• Supporting support at home (SAH ) customers
