Description
The Clinical Care Partner plays a key role in delivering high‑quality, clinically sound, person‑centred services to older people living at home. You’ll provide comprehensive clinical assessments, restorative care management, and expert guidance to Care Partners and consumers across the Support at Home (SAH) Restorative and End‑of‑Life Pathways. This role suits a confident clinician who enjoys autonomy, problem‑solving, and supporting others to thrive. What You Will Do * Clinical assessments and reviews for SAH and CHSP consumers, including urgent clinical interventions when required. * Restorative and clinical case management across the Restorative and End‑of‑Life Pathways. * Provide clinical oversight and guidance to Care Partners and multidisciplinary teams. * Develop individualised care plans and consumer budgets aligned with goals and funding. * Support consumers to navigate services, exercise choice, and self‑manage their care. * Manage admission, review, and discharge processes for SAH pathways. * Participate in MDT meetings and provide clinical helpdesk support during business hours. * Ensure compliance with strengthened Aged Care Quality Standards and SAH guidelines. * Maintain accurate documentation and contribute to continuous improvement. What You Bring * Registered Nurse with current AHPRA registration. * Experience in case management, clinical assessment, and care planning. * Strong understanding of Aged Care Reforms, SAH, CHSP, and quality standards. * Excellent communication, time‑management, and problem‑solving skills. * Ability to work autonomously and respond positively to change. * Willingness to travel across regional Victoria and Northern NSW. * Current Victorian driver’s licence and National Police Check.
