Program Description Reentry Engagement Program is designed to offer clinical and reentry services for adults under the auspices of Adult Forensic Behavioral Health, & Alameda County Behavioral Health and who have been released or expect to be release from Santa Rita Jail and experience behavioral health concerns that are on the Moderate to Severe Mental Health continuum. Clients may include individuals who are homeless or at risk of homelessness, have co-occurring substance use and/or physical health disorders, frequently use hospitals and other emergency services, are at risk of institutionalization, and/or have limited English proficiency. Clinical staff will work closely with AFBH and other County identified partners and/or stakeholders. Felton’s clinical case management team will implement both case management and engagement strategies that are relevant to the situational, cultural and behavioral health needs of the participant. Clinical case management staff shall engage Participants “where they are” with respect to their community location, their need for clinical service/support, and their phase in the recovery process. Job Description The REP Clinical Case Manager will adhere to the principles of Motivational Interviewing, Harm Reduction, Trauma-informed care, Cognitive Behavioral Therapy, Thinking For Change, and Recovery-and-Wellness in order to engage high-need adult probation clients in therapeutic interventions to help identify problems that can have regular occurrences and vary in severity and frequency, thus linking clients with the appropriate social and behavioral health services and supporting them on their path to recovery and re-integration. The Clinical Case Manager will perform comprehensive client assessments, establish Individualized Intervention Plans that will guide service provision, and develop crisis prevention and intervention plans. These activities may include coordinating mental health, social work, client advocacy, educational, healthcare, vocational, housing, transportation, respite care, recreational services and facilitating groups/workshops in the community and maintaining the appropriate caseload ratio. SMI Case Managers will adhere to laws regarding confidentiality and reporting requirements, and will maintain accurate, detailed observation and progress records for electronic data entry. Additional duties may be assigned as needed in order to meet the requirements of the program. Program Specific Responsibilities Works a flexible, full-time schedule to accommodate program/client needs. Some evening and weekend hours are required. This is not a 9-5 position in an outpatient clinic. Provides clinical case management services including screening, evaluations, service plan development, assisting clients with accessing housing, financial, medical, behavioral health and other needed services. Coordinates with other service providers. Provides prevention planning and evidence based interventions and support Conducts aggressive community outreach and engagement in order to safely provide services to clients safely within the community, as needed Maintains client mental health documentation after each client contact or service, in accordance with the county’s mandated standards and Felton requirements. Contributes to team meetings and case conferences through presentation of cases, discussion of clinical issues, and exchange of information and ideas. Other duties and responsibilities may be assigned as required in order to meet program goals and objectives. Minimum Qualifications A minimum of 2 years of case management experience with an adult and TAY population--and substance abuse diagnoses preferred. Master Degree or higher in Social Work, Psychology, Counseling or related clinical field Currently Licensed as an MFT, LCSW, LPCC preferred but not required Registered as either ASW, AMFT or APCC with Board of Behavioral Health Services A minimum of 1 year of experience working with a corrections-involved population preferred Minimum of 1 year substance abuse experience, assessment and crisis intervention skills Experience working with adults who have been incarcerated preferred but not required Ability to work cooperatively on a multidisciplinary team, and to work with clients of diverse ethnic and cultural backgrounds Working knowledge of a variety of computer software applications (i.e., Microsoft Word, Excel, Access, PowerPoint, Adobe Acrobat) Strong communication (both verbal and written) and interpsersonal skills Ability to prioritize projects and follow through Ability to maintain proper documentation as determined by programmatic requirements Ability to be a team player with a welcoming attitude.
