We’re looking for a Clinical Case Manager to join our supportive and welcoming Care team at Amberfield, to integrate and coordinate resident care and service requirements. The Clinical Case Manager (CCM) will ensure that all areas of support for residents and surrounding networks are considered and contacted when organising pre-entry clinical assessments, move-in support, care planning, delivery, and evaluations, referrals, accessing and documenting externally funded care. CCM’s play a critical leadership component within the team, liaising with a wide range of internal and external stakeholders while sharing their knowledge and past experiences to build up and support fellow care staff. If you’re looking to move into a role that works Monday to Friday, 8:30am - 5pm, offering competitive salary rates in the industry plus superannuation and strive to be a positive, welcoming point of contact to our residents, we may just have the role for you!
