About Walden School: Walden School is a nationally-recognized residential therapeutic and education program for deaf children and youth ages 8 to 22 who have often experienced significant adverse childhood events as well as language deprivation. POSITION SUMMARY: The Clinical Case Worker supports the operations of a youth residential treatment program by coordinating cases, scheduling, documenting, and collaborating with interdisciplinary team members. This position does not include direct individual therapy. With appropriate licensure, training, and program approval, the role may expand in the future to include family therapy services. SUPERVISOR: Director of Clinical Services FUNCTIONS AND RESPONSIBILITIES: Clinical Coordination & Scheduling With clinicians, coordinates and oversees the implementation of treatment plans throughout all areas of the milieu. Collaborates with and coordinates support alongside mental health clinicians to ensure continuity and consistency of care. Create and manage clinical meeting schedules in collaboration with therapists. Confirm attendance of participants and update schedules as needed. Coordinate with psychiatrists regarding scheduling of student appointments, observations, and clinical meetings. Transition & Systems Coordination Collaborate and coordinate with the Transition Office to identify appropriate post-residential supports, make referrals, and facilitate appointments and communication with in-state developmental services, including: DMH (Department of Mental Health), DDS (Department of Developmental Services), MassAbility, Deaf, Inc., Walden Community Services, MCDHH (Massachusetts Commission for the Deaf and Hard of Hearing), Other relevant agencies. Support the educational team and family/guardian in completing 688 referrals and other eligibility processes. Collaborate with in-state and out-of-state mental health services regarding eligibility, available services, and care coordination. Support housing arrangements and related services for youth transitioning from residential care. Work with the educational team as students near graduation to plan transitions to adult services, including in-state DDS and out-of-state adult services for individuals with intellectual or other disabilities. Family & Community Collaboration Support family therapy services (as approved and licensed). Coordinate and supervise family visits. Coordinate DCF monthly visits, including communication and scheduling with in-state and out-of-state DCF workers. Serve as a liaison with families/guardians, placing agencies, schools, and community providers. Communicate youth progress, needs, and care coordination updates to stakeholders. Supports on-campus family visits by arranging meaningful activities for families and youth. Additional Responsibilities Support implementation of trauma-informed, developmentally appropriate, and culturally responsive practices. Support coordination during behavioral crises in collaboration with licensed clinicians and supervisory staff. Participate in treatment team meetings, case conferences, and IEP/educational meetings (as appropriate). Modeling therapeutic interventions in the milieu; one night a week, rotating schedule. Requirements Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, Human Services, or a related field – Master’s degree in a clinical field and eligibility for clinical licensure (LMSW, LMHC, LMFT eligible) preferred Experience working with youth in residential treatment, behavioral health, foster care, or related settings Experience working with families, schools, and child-serving systems Knowledge of trauma-informed care, child development, and systems-of-care models Ability to collaborate effectively within multidisciplinary teams Strong professional boundaries and ethical practice Ability to manage competing priorities in a fast-paced residential environment Adept at Google Suite, Microsoft Office: Word, Excel, and PowerPoint Excellent communication and grammar skills at sufficient levels to ensure effective and appropriate communication Strong interpersonal skills developed at a level which allows for the smooth collaboration at all levels within the agency Strong organizational, documentation, and communication skills Periodic flexible scheduling required A valid drivers license and driving record in good standing A minimum of conversational fluency in American Sign Language required, with native, or near-native ASL fluency preferred ASL Classes may be required based on ASL fluency PERK: Meals included when on duty PHYSICAL REQUIREMENTS: Must be physically able to perform all duties of the job including restraining potentially participating in restraining a dysregulated student when necessary.
