FULL JOB DESCRIPTION CLINICAL CASELOAD DEVELOPER:

The Clinical Caseload Developer supports the Director of Rehabilitation and other therapy staff with clinical education, training support, and caseload development to support the clinical and operational benchmarks for each location. Other responsibilities include assuring services are provided in compliance with professional and clinical competency standards; and considering state and federal regulatory requirements.
Reporting lines: Directly to the Clinical Consultant Implementation Specialist

QUALIFICATIONS:

  • Bachelor of Science, Master of Science, or Doctorate Degree in Occupational Therapy, Physical Therapy, or Speech Language Pathology.
  • At least one year experience as an Occupational Therapist, Physical Therapist, or Speech Language Pathologist.
  • Current license in the state of practice.
  • Knowledge of accreditation standards and compliance requirements.
  • Ability to complete the following activities safely, and at times, for prolonged periods:
  • Standing: Maintaining an upright position on the feet, especially for prolonged periods of time
  • Pushing: Applying pressure and exerting force against an object for the purpose of moving it forward or away from the source of the force – push or pull force up to 35 lbs.
  • Lifting: Raising an object from a lower to a higher position, or moving an object laterally, from position to position Required to lift up to 50 lbs. of force independently. May be required to lift over 50 lbs. with assistance of another person or a mechanical lift
  • Stooping: Bending the body forward and down from the waist, hips and knees, sometimes for a prolonged length of time.
  • Reaching: Extending hand(s) and arm(s) in any direction away from the body
  • Balancing: Maintaining body equilibrium to prevent falling when walking, standing or crouching on narrow, slippery or moving surfaces

Responsibilities
  • Lead facilities in the screening process through EMR review and on-site assessment.
  • Assist DORs in the intake process, obtaining consent to evaluate and treat
  • Perform or oversee the initial patient evaluation and evaluate the patient’s response to treatment interventions, as applicable.
  • Understand Sovereign’s Clinical Specialty Programs and assist the rehab teams in implementation and follow through.
  • Understand and follow the regulatory statutes and rules to the practice of rehabilitation services.
  • Attendance at designated meetings, including phone calls, zoom calls, or face to face meetings (including travel time)
  • Development of clinical resources specific to the patient’s diagnosis or deficits, as required for intervention.
  • Regularly communicate with the Clinical Consultant Implementation Specialist, Area Rehab Manager, and Regional Director of Operations to inform them of strategic staffing needs or changes in the facility’s needs/procedures/policies.
  • Provide relevant program knowledge, analytical leadership, administrative guidance to Directors of Rehabilitation and other therapy or facility staff.
  • Provide in-service training related to Specialized Clinical Programs, Sovereign Healthcare Group policies, State and Federal regulations, rehabilitation services, and other approved topics.
  • Participate in facility leadership meetings as needed, including Stand-up, Clinical, Medicare/QAPI weekly meeting, and other related meetings.
  • Coordinate discharge planning and patient equipment needs as needed when supporting patient care.
  • Comply with company, facility, and all other applicable safety rules, policies, and procedures. Implement and ensure compliance with Company, facility, and all other applicable policies and procedures related to the rehabilitation department.
