QUALIFICATIONS

Required:

  • Active Florida Pharmacist License
  • Bachelor of Science (B.S.) or Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited college of pharmacy.
  • Valid U.S. driver’s license and willingness to travel to regional area LTC facilities.

Preferred:

  • Experience in long-term care or geriatric pharmacy is a plus, but we welcome motivated pharmacists eager to grow in this field.
  • Certification in Geriatric Pharmacy (BCGP) or membership in the American Society of Consultant Pharmacists (ASCP) is advantageous but not required.
  • Skills: Strong analytical abilities, attention to detail, excellent communication, a commitment to patient safety and interdisciplinary collaboration, and a desire to make a difference!

Responsibilities

ROLE OVERVIEW

As a Clinical Consultant Pharmacist, you’ll leverage your clinical knowledge to conduct Medication Regimen Reviews, ensuring safe and effective drug therapy for LTC residents. Your work will directly improve patient outcomes by preventing and correcting medication misadventures, collaborating with healthcare teams, and supporting regulatory compliance. This role offers a unique opportunity to focus on clinical care in a non-dispensing capacity, making a lasting difference in residents’ lives.

KEY RESPONSIBILITIES

  • Medication Regimen Reviews (MRR): Our Pharmacists conduct thorough monthly MRRs, analyzing each resident’s full medical record, drug list, lab reports, and progress notes to assess for medication appropriateness, safety, and efficacy. We provide written recommendations directly to prescribers and other professional staff.
  • Clinical Collaboration: We work closely with nurses, physicians, extenders, and facility staff to provide evidence-based recommendations, fostering resident-centered care and positive outcomes. We meet routinely with facility clinicians and staff to review our findings and recommendations.
  • Regulatory Compliance: In addition to our work performing Medication Regimen Reviews, we help facilities ensure compliance with Centers for Medicare and Medicaid Services (CMS) and state regulations, including audits of controlled substances, storage, and documentation. Assist with federal and state survey preparation.
  • Quality Assurance: We participate regularly in facility quality assurance meetings (e.g., QAPI, antibiotic stewardship) and conduct medication pass observations to support high standards of care.
  • Education and Training: We deliver in-service training to facility staff on medication safety, controlled substance protocols, and other clinical topics to elevate care quality.
  • On-Site Visits: Our work is best performed on-site, interacting as part of the team. Guardian Consultant Pharmacists routinely travel to facilities in their geographic area to perform their reviews and provide services, and facilities appreciate us being visible and bringing our knowledge of the industry directly to them! A valid driver’s license and reliable transportation are required.
