Description · Directs the work of the clinical field staff to ensure that effective hospice services are provided, and quality standards are met. Implements corrective actions and conducts performance evaluations as needed. · Provides leadership, direction and guidance to assigned staff. Directs the implementation of Clinical operations, policies and procedures. · Collaborates with medical directors, physicians and other team members to ensure hospice services are performed in an efficient, cost effective manner according to laws, regulations and company policies. · Mentor employees provide input for performance evaluations, counsel and provide disciplinary actions to assigned personnel in conjunction with the HR department, and works to facilitate individual and team development that drives positive results. · Responds promptly to requests from physicians, families, patients, staff and other healthcare professionals to resolve issues and needs. · Supervises staff schedules and delegates client assignment to case managers. Monitors staff productivity daily and assures productivity targets are met. · Oversees client records, statistics, reports and records for purpose of evaluation and reporting of agency activities. Makes provision for the ongoing audit of medical records. · Observes nursing care and visits patient to ensure that nursing care is carried out as directed and treatment is administered in accordance with physician instructions. Evaluates the effectiveness of hospice services and staff competency by direct observation, peer review, and supervisory visits to patient homes. · Assures that clinical staff have the supplies and equipment necessary to provide quality patient care. · Perform visits to patients as needed to include eligibility assessments, such as Start of Cares, Recertification visits, and SN routine visits or supervisory visits. · Effectively addresses personnel issues to promote a productive and healthy work environment, solving complex problems. Reviews and approves clinical staff requests for holiday and vacation time to ensure patient care is provided. · Provides and make provisions for in-service education of clinical staff. Assures that mandatory in-services occur and are attended. · Confers with health team members in interdisciplinary team conference to exchange, discuss and evaluate patient information for planning, modifying and coordinating treatment programs. · Assists the Administrator in preparing for yearly state audit and accreditation on site surveys. Requirements · Licensed Registered Nurse in the State of Oklahoma · 3 years of experience as a professional nurse with 1-2 years administrative/supervisory experience in the field of community health · Excellent communication and time-management skills · Knowledge of regulatory standards and compliance. · Knowledge of techniques and methods of patient care services and instructions for patients and their families · Working knowledge of computer and software applications used in job function (Word, Excel, Outlook and EMR software) · Current Automobile insurance coverage · CPR certified · Workable knowledge of Medicare, CHAP accreditation criteria and standards
