Description JOB SUMMARY:Works within a client and family model of care with acute, chronic and palliative populations within the community and within the context of the vision and values of the organization. The Clinical Coordinator, Home Health, practices within the context of a client and family centred care model, and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and Code of Ethics for registered nurses.The Clinical Coordinator provides leadership, supervision and guidance to designated staff and coordinates work activities to ensure an efficient and high standard of care and effective utilization of client care resources to meet operational needs. Facilitates successful resolution to customer service/client relations issues. Ensures a safe and healthy workplace for clients and staff. Provides clinical leadership and supervision to designated staff to achieve excellence in client care. Assists the Manager with a variety of administrative activities and provides direct health care services to clients as needed. QUALIFICATIONS:Education & Experience Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) year’s recent clinical experience that is relevant to the specific clinical area of employment and two (2) years' recent supervisory experience or an equivalent combination of training, education and experience.
