Job Details Job Location: White Oak of Lancaster - Lancaster, SC 29720 Position Type: Full Time Salary Range: $31.00 - $38.00 Hourly Job Shift: Day Job Category: Health Care DEPARTMENT: Nursing SUPERVISOR: Director of Nursing or Administrator WORK AREA: Entire Facility PURPOSE: To assist the DON with monitoring and overseeing the Nursing Department and to assist in maintaining compliance with state and federal regulations and White Oak Management, Inc. Policies and Procedures. This is a working position that provides / assists with direct resident care. WORKING HOURS: Hours of work are 0 to 80 hours per pay period, plus or minus as business needs and assigned by Director of Nursing or Designee. HIPAA MINIMUM NECESSARY: Responsible to adhere to HIPAA MINIMUM NECESARY guidelines and safeguard protected health information. WORKING CONDITIONS: Works in well-lighted, well-ventilated office and resident areas. May stand, stoop, bend and walk intermittently during the day. May handle emotional disturbances. May be subject to falls, burns, scratches, kicks, and others from residents. May be subject to infectious disease, odors, and personality peculiarities of the aged. When it is not necessary or practical to utilize a mechanical lift, may have to assist with transfer and moving of residents in and out of bed. Must be on constant alert to decrease risk of resident injuries. Is subject to frequent interruptions and emotional overload from both residents and employees. EDUCATION AND QUALIFICATIONS: Graduate of a state approved school of Nursing with current license to practice as an RN or LPN. Current and active license in the state of residence. Experience in geriatric nursing with Charge Nurse or Unit Coordinator experience. Good physical and mental health, considerate, understanding and enjoys working with elderly people. DUTIES: 1. Receives daily report from Charge Nurses, Unit Coordinator, or Shift Supervisor including 24-hour shift report and acute board. 2. Direction exercised: acts as liaison between Nursing staff and the Director of Nursing. 3. Assists in periodic reviews of nursing personnel and daily nursing assignments for continuity of care. 4. Reviews new admission and readmission clinical records as soon as practical after admission or return to facility. 5. Assists Unit Nurse with review of NA (NC)/ CNA (SC) electronic documentation daily. 6. Oversees timeliness of lab work being done as ordered, returns of lab reports and follow-up by Physicians, Nurse Practitioners or Physician Extender. 7. Assists with resident dining program. 8. Attend regularly scheduled Nursing meetings. 9. May assist Staff Development Coordinator with classes, in-services, etc., as needed. 10. Assists with checking daily drug orders for accuracy as assigned. 11. Assists Charge Nurse or Unit Coordinator with floor duty in emergency or as needed. 12. Shares responsibilities with others assigned for monthly drug review, review of resident care guide, and reviewing of physicians orders. 13. Serves as role model for nursing personnel as to personal appearance, dress code, adherence to resident rights, abuse and neglect policies and the safety program. 14. May authorize overtime work of nursing personnel and verify actual time worked by employees with approval from ADON, or DON. 15. Assists with obtaining Responsible Party and/or resident permission for administration of flu/pneumonia vaccine on annual basis as assigned. 16. Acts as Resident Care Coordinator within the guidelines of the facility policies and procedures governing that title. 17. Makes frequent rounds throughout the facility on a daily basis. 18. Demonstrates competency using current clinical software system. 19. Oversees timely completion of electronic transfer/ discharge forms. 20. Oversees No BM Report and follows up daily. 21. Organization of Unit to coordinate work patterns/ resident care needs, twenty-four (24) hours a day/seven (7) days a week. Schedule staff as needed, working with Staff Development Coordinator on procurement of staff, and monitoring of performance of resident care. 22. Keep Unit responsive to resident needs exampled by: Call lights, odors, infection control identification and follow-up, communication as needed with physician and family. 23. Ongoing performance audits of Unit staff to standards of nursing care including: Resident Rights, privacy, standards of care, safety, dietary consumption, and compliance with documentation. 24. Serve as physician contact when possible to explain resident condition, make rounds, process physician orders both in house and out with lab work, X-Ray, and transport. 25. Meet with Unit as indicated to discuss changes, suggestions, etc. for input. 26. Have CNA educated and organized with the following paperwork: weights, intake and output, vitals on residents as needed, mentor program, auditor of PCRs (bowel/bladder, positioning). 27. Coordinate all documentation for Unit, acute changes, ER visits, admissions and discharges, monthly nursing summaries, behavior summaries, accident and incident reports. All must blend together to tell accurate nursing procedures of resident care. 28. Attends the Safety, Infection Control, Quality Assurance, Weights, and Wounds, Care Plan Conferences, and other meetings as needed. 29. Assists with disciplinary action as needed. 30. Responsible for performing QI Audits as assigned by DON. 31. Assist with Investigation of Grievance I/A Reports and obtaining statements as needed. 32. This position is required to take Administrative On-Call as assigned by DON. 33. All other duties as assigned by DON/Admin. #MP2 Qualifications
