Description JOB SUMMARY:In accordance with the Vancouver Coastal Health (VCH) Strategic Framework, and the True North Goals, the Clinical Coordinator, coordinates client care activities to deliver primary care, mental health and substance use services addiction services. Leads in planning and implementation of innovative approaches for delivering quality and efficient services to support/promote health and illness prevention strategies. Supervises and guides designated staff. Consults and collaborates with the interdisciplinary team and family members, and applies the principles of recovery, to provide treatment and care coordination that is both inclusionary and flexible for individuals with mental illness, substance use and concurrent disorders. Acts as a clinical resource for staff, supports staff training and development and promotes education and research. Assists Manager with performing administrative duties and coordinating staffing, scheduling, clinical research, and reporting activities. Provides input into proposals, budgets, policies and procedures. Works collaboratively with the manager, leadership team and other health care providers to promote excellence in client and family centred care delivery within mental health and substance use services. Practices within the context of a client and family centred care model, and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for registered nurses or registered psychiatric nurses, and within the VCH policies, guidelines and clinical practice documents. QUALIFICATIONS:Education & Experience Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM) is required. Three (3) years' recent, related experience in the designated clinical area including one (1) year's related administrative/supervisory experience or an equivalent combination of education, training and experience. Valid BC Driver's License and access to personal vehicle for local area travel may be required.Knowledge, Skills & AbilitiesDemonstrated ability to provide effective leadership, supervision, work direction and consultation.Ability to conduct comprehensive primary care clinical and risk assessments, formulate treatment and discharge plans and provide crisis intervention.Demonstrated knowledge of mental health and addiction housing models and resources.Knowledge of evidence-based best practices and issues to provide primary care, mental disorders, addictions and concurrent disorders treatment.Demonstrated ability to communicate (orally and in writing) and collaborate effectively with clients and their families, coworkers, physicians, other health care staff and staff of external agencies.Knowledge of evidence informed nursing practice related to designated clinical area. Knowledge of legislation and government policies and procedures that relate to the service population.Ability to apply knowledge of theory and practice to a primary care, mental health and addiction process.Knowledge of relevant primary, secondary and tertiary care programs, residential and other community resources.Knowledge of health care disciplines and their role in client care and service provision.Demonstrated ability to develop and deliver educational materials for staff/clients and families based on education theories, principles and best practices.
