The Clinical Coordinator/Nurse Practitioner works in collaboration with the attending physician with the approved protocols established fir the Department Of Addiction Medicine and Adult Primary Care. Provides high-quality health care services including but not limited to the following: a. Performs patient history, physical examination and evaluate findings b. Requests and evaluates diagnostic studies c. Identifies health problems and develop a plan of care d. Implements appropriate treatment including prescription for drugs, devices and immunizing agents e. Provides education to those served f. Makes accurate and complete medical records entries g. Collaborates with other health care providers and makes appropriate referrals h. Provides accurate and appropriate transfer of information when clients are referred to another care provider. -As needed, provides specific addition treatments, medications and follow-up under protocols in the division where assigned. -Provides pre and post test counseling and appropriate services for HIV testing and treatment of AIDS patients. -May be assigned to work in any division of the department. -Performs mental health evaluation, provides summaries of psychiatric status and treatment and assists the attending psychiatrist -regarding treatment for a select patient population. -Consults, refers and collaborates with other members of the Addiction Medicine team involved in the delivery of total patient care. -Participates in peer review and the review of the protocol and procedures -Understands the Diversion Prevention Plan for the Methadone Substance Abuse treatment program and Medically Managed  Detoxification Unit.   Qualifications Current valid license to practice nursing in New York State Certification as ANP, FNP, or PNP with prescriptive privileges by New York State ANA certification in Adult Family Practice preferred.
