The Clinical Coordinator provides operational management and is responsible for the day to day coordination of clinical service delivery and the provision of clinical support to the Complex Care and Chronic Disease Stream, predominantly supporting the complex aged stream. As a Care Coordinator, you will facilitate client self-management strategies in order to improve health outcomes and prevent avoidable hospital presentations. Intervention is provided through a collaborative approach that integrates traditional hospital based health care services and primary health care. Coordinate the day-to-day operation of the service Work collaboratively with the manager to facilitate an engaged, effective and proactive team. Ensure the team provide care in accordance with SVHM HIP model of care, specific service guidelines and where applicable evidence-based practice guidelines. Provide structured clinical reflective practice and operational supervision with team members including performance review. Provide clinical expertise and leadership Direct provision of high quality and evidence-based care coordination, clinical assessment, intervention, education and management for a small caseload of clients (according to profession and skill base). Classifications for this position will range from Registered Nurse Grade 4B, Registered Psychiatric Nurse Grade 4, Allied Health Grade 3. The key aim of CC&CD service is to improve health outcomes for people who either frequently present, or who are at risk of presenting to hospital, because of complex medical and or social issues. CC&CD currently consists of multidisciplinary teams based at SVHM Fitzroy and North Richmond Community Health Hub. Clinical service delivery is undertaken in the clients’ own home and their local community.
