At Transitions Hospice, our Director of Nursing is that leader. This is a role for a seasoned RN who's ready to step beyond the bedside and shape the clinical culture of an entire region. You'll guide interdisciplinary teams, drive quality outcomes, and ensure that every patient in our care receives the compassionate, dignified experience that defines the Transitions difference. What You'll Do As Director of Nursing, you'll own the clinical integrity of regional hospice services — from care planning and quality improvement to team development and operational execution. Your responsibilities span four key areas: Clinical Leadership Provide expert guidance to interdisciplinary teams in developing and individualizing patient care plans focused on palliative comfort Assess and authorize changes in Levels of Care based on evolving patient needs Facilitate nursing interventions and provide direct patient care when necessary Collaborate across the IDG to ensure continuous, coordinated, and well-documented care Quality & Outcomes Lead investigation and resolution of quality issues in partnership with the Continuous Quality Improvement Committee Drive the patient quality metrics that fuel your quarterly performance incentive Evaluate care plan effectiveness and implement improvements that raise the standard of care across your region Education & Mentorship Serve as a clinical mentor and educator for nurses, volunteers, caregivers, and community health partners Foster a culture of continuous learning and professional growth within your teams Promote patient and family education to support continuity of care and meaningful end-of-life outcomes Operations & Team Management Partner with the Administrator to develop and implement manpower and operational budgets Oversee hiring, supervision, evaluation, and coaching of hospice clinical staff Build a team environment grounded in accountability, compassion, and excellence Champion Transitions Care's goals and mission across all levels of the organization Benefits We Offer Competitive pay Quarterly Bonus Opportunity Medical / Dental / Vision Insurance plans Long & Short-Term Disability 401(k) with company match Auto Allowance & Gas Card Company provided equipment Paid Time Off Why Choose Transitions Hospice? Ready to manage with purpose? Apply today Requirements What You Bring RN licensed in the state employed Bachelor’s degree in Nursing (BSN) preferred C.H.P.N. (Certified Hospice and Palliative Nurse) certification preferred. Five (5) + years general nursing preferred with current Hospice, home health, medical, surgical, or critical care experience. Experience with pain and symptom management interventions preferred. Demonstrate excellent observation, assessment, nursing judgment and communication skills. Strong ability to multitask, and prioritize patient needs. Proven leadership experience managing interdisciplinary clinical teams
