An exciting opportunity for a motivated mental health practitioner, who has experience of working within mental health services.
If you are passionate about providing clinical leadership to ensure collaborative work with community care coordinators, First Response Services and acute inpatient/Home Treatment Teams supporting people to further their recovery, and to ensure people only stay in hospital as long as they need to - this is the role for you.
We are seeking someone with a can-do attitude, to join our Discharge Facilitation Team, working alongside Patient Flow. The Discharge Facilitation Team Clinical Lead role is a crucial role across acute care services, identifying and removing barriers to discharge.

Responsibilities

MAIN DUTIES OF THE JOB

The Clinical Lead will provide expert clinical guidance acting as a point of reference for complex clinical cases, offering advice and ensuring high quality care is delivered.
The post holder will act as a crucial link between frontline clinical practice and service management, ensuring the delivery of high-quality, safe and effective mental health care while fostering a supportive and developing environment for the team.
