We are currently recruiting for a Clinical Package Manager to lead and oversee the delivery of person-centred, high-quality care for an individual with complex needs based in Whitby.

Responsibilities

ABOUT THE ROLE:

As Clinical Package Manager, you will work closely with Team Leaders and frontline staff to ensure safe and effective care delivery within the person’s own home, in the local community, and during any hospital stays. Your role will involve clinical leadership, hands-on care, and collaboration with families and healthcare professionals to ensure the best outcomes for the person we support.

KEY RESPONSIBILITIES:

  • Deliver and coordinate person-centred care, ensuring dignity, choice, and independence
  • Provide direct clinical care as needed, including occasional weekends
  • Develop and review clinical care plans and ensure they are implemented effectively
  • Oversee medication management in line with current legislation
  • Lead on infection prevention and hygiene practices within the package
  • Ensure compliance with Care Quality Commission (CQC) standards and internal policies
  • Collaborate with healthcare professionals to meet clinical needs effectively
  • Support and guide Personal Assistants and Nurses, ensuring quality care delivery
