Location: LOWER MIDLANDS/HOME COUNTIES: Bedfordshire, Northants, Buckinghamshire or willingness to travel to these areas approx. 3+ days per week
Hours: 40 hours per week Core Hours to be worked between Monday to Friday. However, your business is 24-hour, 7 day a week service, and your shift pattern will need to reflect the needs of this business requirement and can include occasional night work.

ABOUT US

MCS Healthcare (MedGen Community Services Healthcare) provides personal care and support to people in their own homes to keep them as independent and active as possible. The support we provide is delivered under 2 provision types:
Complex Care which provides personal care to people with complex physical, behavioral or mental health needs. This care is Nurse led and delivered by highly trained and experienced care staff.
Non-Complex Care which provides enabling support to develop and enhance independence skills. This care is led by a Case Manager who has extensive experience and knowledge in health and social care.

Responsibilities

ABOUT THE ROLE

The role of the Clinical Lead is to have overall responsibility for the clinical aspect of all packages, supporting and mentoring the teams who work daily in the packages of care.

KEY RESPONSIBILITIES

  • Be physically visible and accessible to service users, families and carers working together to ensure efficient, effective, and caring services
  • Manage and maintain a caseload of clients within the community
  • Manage, monitor and audit all clinical records such as MAR charts
  • Devise, implement and evaluate care plans, risk assessments and associated care documentation, whilst maintaining a person/care centered approach
  • Continued accountability for care plans, ensuring they are relevant, up to date, person-centered and accurate
  • Work alongside and support the Registered Manager to ensure all referrals are reviewed and assessed
  • Engage and enable our healthcare professionals to effectively advocate the rights and needs of the people in our care, quickly escalate concerns or risks through the correct channels whilst always maintaining professional boundaries throughout
  • Build professional relationships and work collaboratively with key stakeholders
  • Work closely with the Registered Manager to support the continual improvement of Community Care
  • Support, Monitor and be accountable for delegated clinical tasks to Healthcare Workers
  • Provide clinical leadership, supervision and direction to Healthcare Workers to carry out enhanced clinical care duties; maintaining their clinical training and competencies
  • To manage all referrals as directed by the Registered Manager
  • Proactive management and communication to ensure safe, responsive and effective care is provided to reduce risks
  • Support Registered Manager with debriefing service users and carers following critical incidents and difficult situations
  • Support Registered Manager with managing complaints and incidents
  • Be responsible for own Continuing Practice Development (CPD) and ensure training is valid and in date
  • To play an active role when recruiting for positions for specialist care (when applicable)
  • Delivering training where appropriate to support business need
  • Undertaking clinical investigations where delegated
  • Attend meetings as required e.g. Branch, Clinical, MDT, Team meetings
  • Have a flexible and agile approach to meet the needs of the business
  • Provide support and guidance out of hours via on call
  • Any other duties assigned to you by your line manager to fit the business needs
