Job Description: Clinical Lead (Registered Nurse) St Vincent's Virtual & Home programs are growing, providing the opportunity for more people to receive high quality clinical care at home. Integrated with the St Vincent’s hospital and community care network, we are an experienced, supportive and dynamic multidisciplinary team that supports individuals to leave hospital sooner, remain in their homes and achieve their goals. Guided by research, we are embracing new technology, new methods of service delivery and new ways of working. About the Role We are looking for an enthusiastic Registered Nurse with management with hospital in the home experience. This role will lead the establishment of a private SVHA HITH service in Sydney. On a daily basis you will: Lead the establishment and ongoing management of a centralised Hospital in the Home (HITH) program for St Vincent’s private patients. Provide effective leadership to high performing teams ensuring excellence in clinical practice, patient care and patient experience Develop and oversee processes that support the referral of patients into HITH and ongoing coordination of clinical care and resources Provide high quality nursing services delivered within professional boundaries with regard to the assessment, care planning, treatment and review of patients Contribute to the development and delivery of care solutions with a focus on supporting and integrating emerging models and innovations Drive improvement initiatives and service development activities to support the growth of the program nationally Work collaboratively with a wide range of stakeholders including St Vincent’s colleagues and external service providers Your Contribution Minimum 5 years postgraduate experience Minimum 2 years nursing leadership experience in acute, emergency and/or community setting Current nursing registration with AHPRA, as well as a valid driver’s license and access to a reliable car Support an environment of excellence that empowers all team members to be active participants.
