JOB SUMMARY
The Clinical Liaison initiates and develops relationships with current and potential referral sources and evaluates patients for post-acute care services. The liaison performs pre-admission assessments to determine patient candidacy for admission to the inpatient rehabilitation center. The liaison works collaboratively with physician and nurse leaders to ensure compliance with Medicare guidelines and proper documentation both on pre-admission and throughout the patient’s rehab stay.
QUALIFICATIONS
Completes preadmission assessments timely, thoroughly, and with sufficient detail to justify a rehab admission.
Coordinates admission process with referral source, patient/family and rehab team through effective communication and documentation.
Educates potential patients and families, as well as referral sources on inpatient rehabilitation and the benefits of the program.
Develops and maintains positive business relationships with key referral sources.
Develops and implements strategies to grow market share in the core business line in the assigned hospital.
Promotes BSW services and programs to the external community and is available to assist with health fairs, educational programs as needed.
Works closely with service line leadership and other Clinical Liaisons to promote rehab services by participating in educational programs, meet and greets, and other interdisciplinary meetings in hospitals or other levels of care.
Ensures compliance with Medicare guidelines and proper IRF documentation both on pre- admission and throughout the patient’s rehab stay.
Understanding of the IRF-PAI (Inpatient Rehabilitation Facility-Patient Assessment Instrument).
Provides exceptional customer service by ensuring patient referrals are managed and completed in a timely manner.
