Description Facility: Sylva Valley Health & Rehab Job Type: Full-Time Schedule: Monday–Friday with some evenings/weekends/on-call for admissions as needed About the role Sylva Valley Health & Rehab is hiring a Clinical Liaison RN/LPN to support admissions, referral development, census growth, and smooth transitions into our skilled nursing facility. This is a true mix of clinical admissions review and relationship building. The right person will be comfortable reviewing referrals, communicating with hospitals and families, and representing Sylva Valley professionally in the community.
