Description Facility: Sylva Valley Health & Rehab Job Type: Full-Time, Salaried Schedule: Monday–Friday during active referral hours, with remote evening and weekend referral coverage as needed Reports To: Administrator About the Position Sylva Valley Health & Rehab is hiring a motivated, responsive, and relationship-driven Clinical Liaison RN/LPN to lead our referral review and admissions process. This is a dedicated Clinical Liaison and admissions position with no routine floor-nursing assignments. The primary focus is reviewing referrals quickly, making appropriate admission decisions, following every opportunity through completion, and helping grow the facility’s census. The successful candidate will be physically present at the facility each weekday while also completing hospital visits, bedside evaluations, community outreach, and remote referral management as needed. This position is ideal for an RN or LPN who is highly organized, clinically confident, persuasive, and comfortable taking ownership of the entire referral process. Primary Responsibilities Review new referrals and respond within approximately 15–30 minutes Serve as the primary contact for urgent evening and weekend referrals, generally handled remotely Independently approve routine admissions that meet facility guidelines Escalate unusual, questionable, or high-risk referrals to the Administrator for final review Obtain missing clinical records, medication lists, therapy notes, insurance information, authorizations, and other documents needed to make timely admission decisions Communicate admission decisions, acceptance requirements, and outstanding needs directly to hospitals, referral partners, residents, and families Follow up daily on every pending referral until it is admitted, denied, or formally closed Document the reason for every denied, delayed, withdrawn, or lost referral Coordinate smooth transitions from hospitals, homes, assisted living communities, and other care settings Communicate directly with prospective residents and families to answer questions, explain facility services, build trust, and support admission decisions Conduct bedside evaluations and meet with prospective residents and families when needed Provide facility tours to prospective residents and families Assist with verifying insurance coverage, payer information, authorization requirements, and financial information in partnership with the admissions and business office teams Maintain an accurate referral and outreach tracking log, including contacts, follow-up activity, outcomes, barriers, and lost opportunities Provide regular referral updates to the Administrator, DON, admissions team, and other facility leaders throughout the day Build professional relationships with hospitals, case managers, discharge planners, physicians, providers, home health agencies, hospice organizations, and other referral partners Visit hospitals and referral sources as needed Confidently and professionally ask referral partners for appropriate admissions Work closely with nursing, therapy, social services, admissions, business office, and administration Identify referral trends, admission barriers, missed opportunities, and strategies to improve referral conversion and census growth Remain accountable for referral response times, completed follow-up, conversion rates, admissions, and census growth Qualifications Current RN or LPN license Strong clinical judgment and the ability to review referral information independently Ability to manage multiple active referrals without missing follow-up, documentation, or deadlines Excellent communication, organization, customer-service, and relationship-building skills Persuasive, confident, and comfortable discussing services and asking for admissions Able to communicate professionally with hospital staff, referral partners, residents, and families Comfortable working with urgent admissions needs, performance expectations, and census-growth goals Dependable, self-directed, and able to make timely decisions Professional appearance and conduct Reliable transportation for hospital visits, bedside evaluations, and referral-source outreach Ability to be physically present at the facility each weekday Ability to provide remote evening and weekend referral support when needed Admissions, skilled nursing, long-term care, post-acute, hospital, marketing, sales, or clinical liaison experience is preferred but not required.
