REQUIREMENTS:

  • Minimum 1 year of Home Health experience
  • At least 1 year of management experience in a clinical setting
  • 2-3 years of RN Nursing experience (managerial experience preferred)
  • Strong background in patient care coordination and escalation management
  • Proficiency in clinical coordination and regulatory compliance
  • Demonstrated experience in re-hospitalization reduction, resource management, and staff training
  • Knowledge of all relevant federal, state, and local regulations for home healthcare
  • Strong organizational, communication, and leadership skills
  • Ability to adapt in a dynamic environment
  • A valid RN license in California is required
Responsibilities
  • Lead, supervise, and support a multi-disciplinary team to deliver top-quality patient care and ensure patient satisfaction
  • Conduct patient visits as necessary to maintain oversight of care quality
  • Stay up-to-date with all federal, state, local, and Medicare regulations relevant to home healthcare
  • Oversee new admissions, execute physician orders, and create comprehensive care plans for patients
  • Manage agency readiness for surveys and compliance with regulatory standards
  • Assess and address staffing needs, creating achievable goals to improve services and expand our referral base
  • Conduct interviews, recruit clinical staff, and ensure alignment with agency standard
  • Monitor and manage re-hospitalization cases, productivity, and territory assignments
  • Provide leadership support to clinicians, ensuring proper coordination and protocol adherence
  • Ensure compliance with referral partner protocols and resolve complaints as needed
  • Work closely with case managers and marketing teams to strategize referral generation
  • Coordinate training for new hires, both in-office and in the field
  • Manage staff performance, provide feedback, and handle disciplinary actions when necessary
  • Collaborate with HR and other departments to implement training programs and resolve staffing needs
  • Stay informed on industry trends, and work to continuously improve patient outcomes and satisfaction
