Position Summary: We are seeking a dynamic and experienced Clinical Manager (RN) to lead our field staff and oversee the delivery of skilled and non-skilled private duty home care services. This role ensures the highest standards of care through effective planning, coordination, supervision, and evaluation of clinical operations. The Clinical Manager serves as a key liaison between patients, families, staff, and interdisciplinary teams. Key Responsibilities: Lead and manage a team of field clinicians to ensure quality, compliance, and efficiency in home care delivery. Supervise and support staff through hiring, coaching, training, and performance evaluations. Perform and oversee Start of Care (SOC) visits as needed to ensure timely initiation of services and accurate assessment of patient needs. Oversee patient care plans, recertifications, and discharge processes. Conduct multidisciplinary case conferences and ensure timely, accurate documentation. Monitor staffing levels and scheduling to meet patient needs and agency standards. Serve as a resource for clinical policies, procedures, and regulatory guidance. Perform in-home supervisory visits and provide direct care when necessary. Collaborate with leadership on performance improvement initiatives and staff development. Requirements Qualifications: Experience: Minimum 2 years as a registered nurse in home health; 1 year in a supervisory or management role. Skills: Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and ability to communicate verbally and in writing effectively. Demonstrates proven decision making skills. If required to make visits, Hepatitis profile. Must read, write and comprehend English. Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. Licensure/Certification: Current driver’s license in good standing. It is the responsibility of the employee to renew their driver’s license before it expires in order to continue employment. Current State professional license as an RN in any/all applicable states. Current CPR Physical Requirements: Prolonged sitting required in an office environment which involves minimal and occasional lifting of medical supplies and charts. If visiting patients, prolonged standing and walking required, with the ability to bend, stoop, reach and move client weight up to 250 pounds as well as carry up to 30 pounds. The ability to access clients’ homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client’s condition and to perform client care. Ability to handle stressful situations in a calm and courteous manner at all times. Requires working under some stressful conditions to meet deadlines and Agency needs. Environmental/Working Conditions: Performs duties in an office environment during Agency operating hours. Ability to work flexible schedule. Ability to travel locally. Some exposure to unpleasant weather. If required to make patient visits, may work in patients home in various conditions; possible exposure to blood and bodily fluids and infectious diseases.
