In this role, you will be responsible for providing comprehensive coordination for patient care in designated specialties. Your focus will be on fostering communication among care teams, patients, and families to ensure a seamless healthcare experience. By promoting integrated care and evidence-based practices, you will play a crucial role in optimizing patient outcomes and driving clinical excellence. Key Responsibilities:


- Serve as a point of contact for patient education and referrals

- Collaborate with physicians and healthcare professionals

- Track patient progress throughout their care continuum

- Facilitate communication among interdisciplinary team members

- Engage in community education and outreach initiatives


Requirements:


- Valid Registered Nurse license

- 2 years of RN Pediatric Acute Care experience

- Bachelor’s Degree in Nursing required

- Master’s Degree in Nursing preferred

- BLS certification must be current
