In this role, you will be responsible for providing comprehensive coordination for patient care in designated specialties. By promoting integrated care and evidence-based practices, you will play a crucial role in optimizing patient outcomes and driving clinical excellence. Key Responsibilities:

- Serve as a point of contact for patient education and referrals

- Collaborate with physicians and healthcare professionals

- Track patient progress throughout their care continuum

- Facilitate communication among interdisciplinary team members

Requirements:

- Valid Registered Nurse license

- 2 years of RN Pediatric Acute Care experience

- Bachelor’s Degree in Nursing required

- Master’s Degree in Nursing preferred

- BLS certification must be current
