ABOUT THE ROLE
The Acute & Continuing Care Team (ACCT) provide mental health support for adults (18-64) who are experiencing a mental illness. This role is based at the Curran Centre and provide in reach into Orange Health Services Emergency Department.
This role involves providing timely, safe and comprehensive mental health triage and assessment for patients presenting to the Orange Health Services Emergency Department as well as to the ACCT team. This includes the timely transitions of patients from the Emergency Department into the care of the Community Mental Health Services and/or other service providers, providing clinical documentation, liaison and follow up necessary to ensure safe and efficient and effective continuity of care. The CNC may also provide Clinical Liaison support to the Lachlan Community Mental Health team.
Within the practice this role provides clinical services and consultancy, clinical leadership, education and participate in clinical services planning and management.
