You’ll assess patients 65+ within the Hornsby Ku-ring-gai catchment, focusing on reducing re-presentations and enhancing discharge planning. This is a unique opportunity to build expertise in aged care within a dynamic, 7-day service. The role will work closely with Rehabilitation Aged Care Service and the Emergency Department at Hornsby Ku-ring-gai Hospital.
