The Clinical Nurse Consultant (CNC) is an expert practitioner who, through leadership facilitates evidence based practice. The CNC will promote a service which fosters clinical excellence, research and professional leadership. The CNC contributes to quality health care by collaborating with the health care providers and consumer groups, developing clinical practice standards and evaluating their outcomes. The CNC is responsible for providing services which facilitate effective management of aged care people to provide care close to home and prevent avoidable presentations and admissions to hospital. The CNC will provide triage for the service in collaboration with the Geriatrician. The CNC will support education to other nursing staff in COPILOT and in Residential Aged Care Facilities to ensure high quality care delivery. Their role will include research and service development in this new urgent care service.
